Responsibilities
Support planning and coordination of the CPHT program and its activities
Coordinate and maintain applicable program schedules and agendas, including tracking deadlines, deliverables, and communication with management and stakeholders
Conduct door-to-door surveys and assessments in the assigned areas of the San Gabriel Valley
Distribute over-the-counter medical kits in the assigned areas of the San Gabriel Valley
Deliver general welfare interventions to the assigned communities of the San Gabriel Valley
Use software apps and tools to collect data
Save and report the collected data
Perform other duties as assigned by the Operations Program Manager

Required
Bachelor's degree in public health or health sciences, or at least three years equivalent experience in relevant field
Minimum 1 year experience in public health, healthcare settings, community outreach or related field
Experience in community outreach, conducting surveys, collecting data and reporting data
Experience working with community-based organizations, local government agencies, volunteer groups in LA County preferred
Ability to work independently and as part of a team
Excellent written and verbal communication skills
Proficient in MS Office applications
Flexible and adaptive to a frequently changing environment
Excellent organizational and problem-solving skills
Close attention to detail and a high level of accuracy
Bilingual in English Spanish/ and/or Mandarin required
Ability to work outside for 36 hours a week
Ability to walk 4-5 miles/day (reliable transportation required)
Knowledgeable of HIPPA
