Responsibilities

Supervises a team of staff, providing guidance on problem-solving, crisis management, time management, and client barriers to care.
Supports program staff to problem solve and work collaboratively to address most needs and challenges that happen at sites.
Oversees and provides outreach and case management services including intakes, assessments, SDOH forms, basic service planning, referral and linkage management, and advocacy to and for individuals who are homeless, experiencing SMI and/or SUD, or are justice impacted. Supporting participants in obtaining government benefits such as Medicaid, SNAP, etc.
Coaches team on harm reduction, motivational interviewing, trauma-informed care, basic case management skills, and other evidenced-based practices.
Maintains a networking relationship with community agencies, clinics, juvenile and adult justice systems, and supportive services for referrals/linkages and ancillary services.
Support staff with caseload management, including covering shifts and client appointments as needed.
Train staff on various housing waitlist resources, including Coordinated Entry System, SRN, CHA, site-based HUD section 8, SROs, and more
Actively participates in relevant networks/coalitions serving clients affected by SUD, SMI, justice involvement, and homelessness, and represent HAS at community meetings and coalitions.
Manages linkage agreements with external stakeholders, ensuring accuracy of linkage agreements to employee portal and responsible parties across the agency are able to ensure updates in a timely fashion.
Manages reporting and documentation in CareLogic and ensures timely staff documentation. Runs regular reports to monitor data entry.
Monitors grant scopes and collaborates with Program Director and Development to ensure timely and accurate reporting.
Provides anti-stigma education to the community and individuals dealing with mental illness and/or substance use disorders as well as information on prevention, treatment, and recovery for participants.
Meets regularly with the Program Director as a means of enhancing professional growth, reviewing program/grant scopes and goals, and dealing with appropriate administrative issues.
Performs other duties as assigned by the Program Director.

Required

Work will require traveling and will require the candidate to be onsite.
Strong organizational, communication, and critical thinking skills, including the ability to work as part of a collaborative treatment team.
Strong computer skills.
Must be flexible with hours to accommodate participant and staff needs.
Possession of valid Illinois Driver’s license and willingness to use personal vehicle in course of employment, including transportation of participants.
100/300/100 Automobile Insurance.
H.A.S. requires all staff to work onsite.
GED/High School Diploma required, Bachelor’s Degree strongly preferred.
At least two years of leadership and/or management experience.
Have five years of case management experience in a non-profit setting.

Bilingual/Bicultural is preferred (English/Spanish).
