Responsibilities
Enhance SJRH's capacity for opt-out HIV and Hepatitis C testing in Emergency and Inpatient Departments.
Develop systems to link eligible individuals to HIV treatment, prevention, and Hep C treatment through the hospitals EMR system.
Collaborate with ED/Inpatient staff to develop efficient workflows and patient-centered interactions.
Enhance referrals for linkage to care within the hospital's system or community providers.
Enhance the EMR to support access to data for testing and linkage.
Implement spreadsheets/databases for tracking patients linked to care.
Update organizational policies to support universal testing and linkage services.
Support quality improvement efforts for testing and linkage services.
Link HIV-negative and at-risk individuals to Comprehensive Prevention Services.
Ensure grant deliverables are met and program compliance with funders' Scope of Service.
Collect and report monthly/quarterly data elements as required.
Work with the Projects Principal Investigator/Project Champion on system implementation and data-driven presentations.
Supervise Patient Navigator and Data Analyst.
Perform other duties as assigned during emergencies.

Required
Masters in Public Health, preferred
Bilingual: Spanish/English is preferred
Preferred
Masters in Social Work will be considered, based on applicants experience
