Responsibilities

Leads team that is responsible for continuous development of our care model that defines target populations, patient interventions, and intervention effectiveness. Ability to analyze incoming data to identify trends, ensuring our care model is effectively designed and implemented to address patient needs.
Leads team that is responsible for designing functionalities to implement patient interventions, ingesting and analyzing market feedback to optimize care team user experience and intervention scalability.
Leads team that is responsible for deploying upfront training, ongoing quality assurance, and technical assistance to care team members.
Deeply collaborates with Diverge’s Chief Innovation Officer, Market Operations, Technology, People and Finance teams to ensure there is cross-functional alignment on the Population Health team’s ongoing work and roadmap.
Promotes Diverge Health's culture and brand amongst both employees and external partners.

Required

15+ years of experience leading and growing teams in fast-paced healthcare companies.
Significant experience working closely with care teams serving patients on Medicaid and Medicare.
Demonstrates a strong understanding of value-based care principles, including the impact on patient outcomes, quality, and cost.
Extensive experience leading the development and implementation of innovative patient care programs.
Track record of synthesizing vast amounts of information, prioritizing work accordingly with an eye to near-term feasibility, and collaboratively partnering with operations, technology, people, and finance teams.
Strong attention to detail, analytical, effective communication skills, and a collaborative mindset.
An ability to travel to our markets on an as-needed basis.

Preferred

5+ years in management consulting and/or corporate strategy roles, demonstrating a proven ability to produce high-quality deliverables, communicate effectively with executive leadership, and exhibit strong executive presence in high-stakes environments.
