Responsibilities
Implement key quality and population health strategies alongside providers to improve health outcomes
Assist practices in identifying areas of need and enhance efficiency through data sharing and reporting
Coach practices on quality improvement and value-based purchasing goals
Facilitate provider engagement meetings and ensure follow-up on action items
Monitor and present results of quality activities to management and other departments

Required
Bachelor's Degree or equivalent combination of education and work experience
Minimum 3 years of experience in healthcare, with at least 1 year in health plan quality improvement or risk adjustment
Strong business writing skills
Operational knowledge and experience with Excel and Visio
Ability to manage multiple tasks and meet deadlines effectively
Preferred
1 year of experience in Medicare and Medicaid
Certification as a Certified Professional in Health Quality (CPHQ)
Nursing License (RN may be preferred for specific roles)
Experience in Clinical Quality or Public Health
Familiarity with healthcare quality improvement initiatives
