Responsibilities
Serve as the primary liaison with referral sources, coordinating admissions and ensuring financial and medical clearances
Manage the intake process, collecting and documenting necessary patient information accurately and timely
Handle patient referral calls and communications, providing excellent customer service
Collaborate with multiple departments to facilitate patient placements across facilities
Maintain and update Salesforce CRM with current data and status of admissions referrals

Required
High School Diploma required; Associate’s Degree desirable; Bachelor’s Degree strongly preferred
2 – 4 years of related experience in a healthcare or customer service role
Excellent sales and customer service skills, with strong communication and organizational abilities
Detail-oriented with the capability to enter information accurately into database systems
Proficiency in standard office applications, including email, word processing, and spreadsheets
Preferred
Previous experience using Salesforce or EMR systems
Ability to adapt to changes in procedures and policies positively
Strong interpersonal skills to collaborate effectively with various teams and management levels
