Responsibilities
Provide direct supervision and clinical oversight for assigned team.
Ensure staff utilize workflows as required in Trillium Software Platform.
Monitor teams to help ensure integration of care.
Create and identify methodologies and tools to be used in the execution of care management and coordination.
Support team members to learn about and implement different aspects of care management and coordination.
Track the implementation of workflows, policies, and procedures.
Review member information within the platform.
Communicate regularly with team members.

Required
Master’s degree
Three (3) years of experience providing care management, case management, or care coordination to the MH/SU/BH population.
NC fully licensed LCSW, LCMHC, LPA, LMFT, LCAS or RN required.
Must have a valid driver’s license.
Must reside in North Carolina to be considered for remote status.
