Responsibilities

Lead overall project management and oversight of the PHIL PATH CPI project to ensure the project achieves the expected results outlined in the contract.
Serve as point of contact for PHIL internal project teams ensuing team actions remain in synergy.
Ensure the project timeline and scope of work are on time and completed in a quality manner.
Oversee the development and facilitation of monthly regional convenings, working groups, and office hours to identify, discuss, and resolve local ECM and Community Supports implementation issues that arise as CalAIM is rolled out across a county/region.
Demonstrate strong written and verbal communication skills that adapt to different audience and stakeholder types.
Identify Enhanced Care Management and Community Support needs and gaps within the assigned region(s).
Conduct quality improvement activities to ensure the delivery of high-quality services.
Conduct program evaluation activities in partnership with the PHIL Monitoring, Evaluation Research, and Learning Innovations (MERLIN) staff.
Monitor how PATH and other funds are being used to address implementation issues in the PHIL assigned regions.
Identify and develop training content, coordinate trainings and conferences, and deliver trainings.
Curate a summary of best practices utilized and or developed within the collaborative membership.
Provide daily supervision and manage performance evaluation processes for the PATH Program Associate II.
Identify and secure subject matter experts (PATH CPI Consultants) to support collaborative topics of interest and technical assistance to PATH CPI participants.
Manage and supervise PATH consultants results by communicating job expectations; planning, monitoring and evaluating job results; developing, coordinating, and enforcing systems, policies, procedures and programmatic standards.
Serve as the project point of contact for the PHIL PATH CPI project.
Conduct outreach to entities that have been historically underutilized and/or under-resourced (as defined by DHCS), and/or that serve the diverse needs of the state’s population to encourage participation in CalAIM; and/or other activities approved by the State.
Refine and continue implementation of the regional collaborative recruitment, engagement, and retention strategy for the assigned regions.
Identify and invite entities to participate in local collaborative planning groups.
Develop and manage relationships with relevant partners.
Oversee a $1M contract award with support of Learning and Action and PHIL staff.
Ensure appropriate technical assistance across the two PATH regions, lead the process to secure technical assistance providers through subcontracts.
Participate in funder program meetings and networking opportunities.
Develop work plans, status reports and budgets, such as tracking membership and partnership activities, program expenditures and donations.
Travel within the Northern California regions when required, at minimum of 2-4 times per year per region.
Perform other duties as needed in support of the program as assigned by the Program Director and Director of Learning and Action.

Required

7 years of experience in management and/or planning experience working in the non-profit, Public Health, Public Policy, Healthcare administration, or other related field
Bachelor’s degree, or substitute with four additional years of relevant experience
Experience in convening design and facilitation
Demonstrated understanding of Medicaid health policy, including experience, and understanding of Medi-Cal, CalAIM and the California market
In-depth understanding of Medi-Cal Medicaid Waiver policy and the ability to translate to practical implementation and related strategy
Experience and skills in project management; non-profit marketing strategy/implementation; accounting/reporting; fundraising; relationship/coalition-building; curriculum development/instructional design; supervision/people management; program development; media/PR
Demonstrated experience serving in high-level leadership roles with a proven record of successfully implementing and managing complex, multi-sector initiatives
In-depth practical knowledge and hands on experience in the planning, implementing, and tracking of quality improvement and related methodologies
Ability to design and facilitate meetings, build consensus, and work collaboratively
Strong oral and written communication skills, including translating technical material for a lay audience
Strong interpersonal skills – ability to communicate and establish effective working relationships with all levels within the organizations, and interact effectively with co-workers, customers, and other stakeholders
Ability to make independent decisions following standard policies and procedures and to work independently with limited supervision
Ability to convene and engage a diverse set of stakeholders with different backgrounds and points of view
Ability to create a fair, open, and transparent process, and an impartial environment where constructive, disparate points of view can be expressed
Ability to support the identification of challenges and conflicts associated with the implementation of CalAIM
Ability to identify potential resolution strategies and tactics to overcome challenges and conflicts, including identification and dissemination of successful practices to a diverse set of stakeholders
Ability to facilitate comprehensive programming (e.g., webinars, in-person convenings, etc.) that allow for robust collaboration amongst participants
Ability to conduct outreach to organizations that serve historically marginalized populations and other entities to actively participate in collaborative planning groups
Ability to facilitate a team; Strong teamwork skills; ability to balance team and individual responsibilities and support others’ efforts to succeed
Ability to exercise initiative, be creative, and be resourceful
Ability to function in a fast-paced environment, make simultaneous progress on multiple tasks, and meet deadlines
Problem-Solving: ability to identify and resolve problems in a timely manner; ability to develop alternative solutions
Good computer skills including Microsoft Word, Excel, PowerPoint, and internet searches
Have access to reliable transportation and ability to travel within Northern California to the PHIL assigned regions when required, at minimum of 2-4 times per year
Have access to reliable transportation and ability to travel to Sacramento or elsewhere in California when requested to attend funder related convenings 2-4 times per year
