Responsibilities

Conduct in-home health risk assessments for members in your geographic area

Evaluate past medical history, review systems, reconcile medications, and perform vital signs and physical exams

Use evidence-based practice screening, including point-of-care testing as needed

Identify diagnoses for care management by Porter Care Guides

Actively manage medical treatment based on assessment findings

Formulate lists of current and past medical conditions using clinical knowledge and judgment

Qualification

Michigan NP license3 years patient care experienceEMR skillsEmpathyCommunication skills

Required

Valid unencumbered Michigan NP license

3 years of patient care experience required

A strong sense of empathy, compassion, and friendliness

Exceptional communication skills, both written and oral, with the ability to assist others (patients and coworkers alike) with sincerity, respect, and compassion
