Responsibilities

Provides clinical occupational therapy evaluation and treatment on an acute, restorative and maintenance level to program participants
Participates in Interdisciplinary Team (IDT) by contributing insights from assessments, care plan recommendations, and care coordination in a collaborative spirit
Assesses condition, creates a treatment plan and discusses implementation of therapy with participants and caregivers
Instructs family members, caregivers, and staff in methods to maximize safety and functional abilities in performing Activities of Daily Living (ADL) with documentation in medical record notes
Makes home visits as needed and documents activities including participant/caregiver response in participant’s chart
Educates participants and family members in proper care and use of various supports and exercise programs in addition to the use of DME
Documents therapy services provided and performs department record-keeping in keeping within documentation requirements
Contribute to a center experience that Participants want to spend time in, a team culture that cares and creates joy, and an environment where all participants and team members belong
Continue to raise the bar. Constructively seek and share feedback, and help us implement changes in order to improve clinical outcomes and experience for participants

Assists in the ordering, inventory, distribution, and maintenance of durable medical equipment (DME) for participants
Performs data collection, prepares reports and maintains any necessary participant statistics
Attends all staff meetings and patient care conferences as scheduled
Maintains the quality assurance, safety, environmental and infection control programs
Serves as subject matter expert for the development of occupational therapy program standardization, evaluation and team education
Serve as subject matter expert to support quality programs and discipline-specific audits

Required

Bachelors in Occupational Therapy from Accredited Program meeting AOTA & ACOTE standards
National Certification by the American Occupational Therapy Association or eligible for National Registration Examination of the American Occupational Therapy Association
Registration with the state of California
Requires a state issued driver's license, personal transportation, and auto insurance as required by law
A minimum of one year experience working with the frail or elderly
Proof of medical clearance for communicable diseases, including a TB test
Proof of all immunizations are up to date

Preferred

2+ years as a practicing OT
Bilingual: Spanish/Mandarin/Cantonese preferred
Masters in OT (MOT), Doctorate in OT (OTD)
Experience (or demonstrated interest) in clinical program development
