They are seeking a Social Worker for Home Hospice to collaborate with the Interdisciplinary Team, providing social work interventions, education, counseling, and support services to meet the psychosocial needs of patients and caregivers.

Responsibilities
Provide social work interventions, education, counseling and support services to address the identified psychosocial needs of patients and caregivers
Qualification
Required
MSW required
LSW or LCSW must be obtained within two years from date of hire
Must be able to complete 4 - 5 weeks of a FT, day shift orientation
Must work a minimum of 64 hours per two-week pay period
Must work a minimum of 40 hours per two-week pay period
