SUPPORT COORDINATOR, REACH ADULT - RAPPAHANNOCK AREA (FULL TIME)

Region Ten CSB is dedicated to providing support for adults and children with developmental disabilities and behavioral health needs. The Support Coordinator is responsible for crisis response, developing Crisis Education Prevention Plans, and linking clients to necessary services while providing on-call support for crisis situations.

Responsibilities

Serve as a Region 1 Regional Education Assessment Crisis Services Habilitation (REACH) Support Coordinator for adults and children with developmental disabilities and behavioral health needs
Provide crisis response requiring de-escalation, clinical and coordination services, assessments, developing and implementing Crisis Education Prevention Plans (CEPP), linkage to other services, follow-up, and crisis stabilization services
Share responsibility as Primary and Secondary On-Call staff providing a rotating 24/7/365 crisis line, including on-site response

Required

Qualified Mental Health Professional (QMHP or QMHP-T)– minimum of Bachelor's degree in human services or related field from an accredited institution and no less than one year of full-time relevant experience
Valid Virginia Driver's License plus an acceptable driving record as issued by the Division of Motor Vehicles is required
Knowledge Of characteristics and service needs of persons with intellectual and developmental disabilities, mental illness, behavioral disorders, language disorders, and other sensory impairments
Knowledge Of behavior modification; Cognitive Behavioral Therapy (CBT); assessment and intervention strategies; emergency interventions; family, group, and individual therapies; biopsychosocial education; risk assessments; sex offender treatment; substance abuse interventions; etc
Knowledge Of community mental health and public health in order to provide advocacy and enhance individual's treatment and access community resources
Knowledge Of principles, procedures, techniques, trends, and literature related to mental health, intellectual disabilities, DD, and substance abuse
Knowledge Of psychiatric/medical medications/treatments
Knowledge Of assessment and intervention strategies with persons with severe, multiple disabilities (counseling, training, etc.)
Knowledge Of non-aversive behavioral management techniques/crisis intervention skills
Knowledge Of data collection capabilities
Knowledge Of specific local, state and federal regulations
Knowledge Of philosophies of normalization/ integration
Skills In consumer needs assessment, service planning, development of habilitative programs and evaluation of individual progress
Skills In general knowledge of counseling and behavioral management skills with dually-diagnosed persons
Skills In general knowledge in the management of aggressive behaviors
Skills In organizing records
Skills In service delivery
Skills In negotiating services with contractual and other agencies
Skills In managing medical emergencies
Abilities To adhere to established policies and procedures
Abilities To analyze situations accurately and develop clinically appropriate responses and/or interventions
Abilities To communicate effectively as part of an interdisciplinary team in relaying information to facilitate treatment
Abilities To perform clinical interviews in order to obtain information and to formulate appropriate treatment plans
Abilities To develop reports, create presentations, and communicate findings to both internal and external audiences
Abilities To utilize database systems and software to efficiently and effectively meet programming requirements and needs
Abilities To demonstrate flexibility in working modified schedules as required
Abilities To demonstrate willingness to drive and must possess a valid driver's license

Master's Degree in a human service field (counseling, special education, psychology, social work, or closely related field)
