Responsibilities
Review patient records to ensure that information is accurate
Submit Review Visit Logs (RVLs) to supervisor
Adhere to the operational guidelines for color coding of patients
Respond to HR, operational, and clinical correspondences
Submit clinical corrections to the supervisor
Attend all operational and educational pod meetings as scheduled by the pod leader and submit the post-educational survey following these meetings
Promptly communicate with Operations to resolve any IT issues encountered
You will be cross-trained in other areas of care management
Qualification
Required
LPN certificate/ degree
Ability to work full-time
