The Transitional Nurse will support new case starts, urgent callouts, and complex care needs while collaborating with multidisciplinary teams.

Educate and support families and caregivers

Qualification

Active LPN licenseTrach/vent experienceGraduation from accredited programClinical skillsProactive mindsetComfortable with pediatric clientsComfortable with adult clientsFlexibility

Required

Active LPN license in the state of New York

Graduation from an accredited nursing program

1+ year of trach/vent experience (home care preferred)

Comfortable working with both adult and pediatric clients

Willingness to travel to 3 out of 4 areas; The Bronx, Westchester County, Rockland County, Orange County

Strong clinical skills, flexibility and a proactive mindset
