Internal Medicine Nurse Practitioner
Lone Star Circle of Care is a non-profit organization in Central Texas focused on providing high-quality health care to underserved populations. They are seeking an Internal Medicine Nurse Practitioner who is passionate about improving access to primary healthcare and is comfortable providing care both in person and virtually.

Provide care both in person and virtually to underserved populations ages 55+
Work with patients to decide the best course of treatment in an outpatient setting
Interact and collaborate with community partners and emergency medical/psychiatric service providers
Participate in after hours on-call rotation 3-4 times a year
Document within an electronic health record and conduct tele-medicine visits
Qualification
Required
You are a board-certified APRN who is passionate about working with the underserved population ages 55+ who may be experiencing significant barriers to care
You are comfortable providing care both in person and virtually
You want to grow a practice where you and your patients work together to decide the best course of treatment in achieving their goals in an outpatient setting
Mission focused and passionate about improving access to quality primary healthcare
Proactive and thrives in a fast-paced clinical setting
Comfortable working independently in a group setting while also seeking support and consultation from colleagues
Current valid license to practice in the State of Texas
Ability to interact and collaborate with community partners and emergency medical/psychiatric service providers to ensure patient safety if a patient needs a higher level of care
Participate in after hours on-call rotation 3-4 times a year
Possess computer/technology skills to efficiently document within an electronic health record and conduct tele-medicine visits, as well as communicating/meeting with colleagues virtually
