Coordinate and complete MDS assessments accurately and timely
Collaborate with healthcare providers to gather information for assessments
Ensure compliance with state and federal regulations related to MDS assessments
Review and update resident care plans based on MDS assessments
Communicate assessment findings to appropriate healthcare team members
Participate in quality improvement initiatives related to MDS assessments
Maintain accurate and up-to-date records of assessments and care plans
Qualification
Required
Bachelor's degree in Nursing or a related field
Active Registered Nurse (RN) license
Previous experience in MDS coordination or a similar role in a healthcare setting
Knowledge of MDS assessment process and regulations
Strong attention to detail and organizational skills
Excellent communication and interpersonal abilities
