Responsibilities

Conduct thorough initial assessments, including comprehensive evaluations of social determinants of health, medical history, and mental health status
Provide compassionate care and emotional support to patients, employing effective communication strategies tailored to individual needs
Utilize nursing assessment skills to identify and address complex health issues among diverse patient populations, including those experiencing homelessness and individuals with histories of substance abuse or mental illness
Implement evidence-based practices and harm reduction techniques to mitigate health risks and promote overall well-being
Collaborate with interdisciplinary teams and community resources to facilitate access to healthcare services and support programs for underserved populations
Advocate for patients' rights and address systemic barriers to healthcare access and equity

Required

Graduate of an accredited LPN program
Active Massachusetts licensure as a Licensed Practical Nurse
Basic Life Support (BLS) certification required
Minimum of 1 year of nursing experience in is preferred
Strong nursing assessment skills, particularly with diverse populations such as individuals experiencing homelessness, those with histories of substance abuse or mental illness
Ability to learn quickly and adapt to different healthcare environments
Strong communication and interpersonal skills

Preferred

Experience working with underserved populations
Proficiency in multiple languages is highly desirable
