Responsibilities

Provide skilled services to evaluate and address the needs of individuals in their homes

Assist with home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, management of chronic diseases and dementia family caregiver training

Serve as an educator, patient advocate and a member of the interdisciplinary care team
