Responsibilities
Completing assessments (admission assessments for rehab and long-term care, depression screening, trauma screening)
Coordinating services for discharge planning
Attending and assisting with team/family meetings and care planning
Providing emotional and social support for residents and families
Qualification
Required
Currently enrolled in a Bachelor's or Master's degree program in Social Work, Human Services or a related field
Excellent communication and interpersonal skills
Ability to work effectively both independently and as part of a team
Ability to maintain confidentiality and handle sensitive information with professionalism
