Responsibilities

Recruit, train, and lead a team of high-performing APPs delivering high-quality care to our patients

Monitor quality of care and performance across a team of APPs, identifying training opportunities at both the individual and team level. Work with nurse educators and care operations to roll out additional trainings across the APP team

Support the team in addressing barriers to care that have been identified by patients and/or the care team

Hold the team accountable for improving outcomes through increasing patients’ uptake of our population health programs and protocols

Provide insights about frontline care and opportunities for process improvements to leadership and multi-disciplinary stakeholders

Identify opportunities for professional growth for each individual on the team and support them in working toward those goals

Partner closely with clinical leadership across the organization, including Medical Directors and Heads of Nursing & Specialty programs

Able to step in to support patient care when needed

Qualification
