As an on-site Nurse Practitioner, you will provide direct patient care while integrating technology into clinical workflows, aiming to reduce preventable readmissions through proactive health interventions.

Responsibilities
Conduct initial and follow-up assessments on flagged residents; initiate/adjust plans of care; complete consent; coordinate with facility providers
Train and coach staff at the point of care, standardize use of Circadia into clinical workflows, and run live demos during real patient encounters
Interpret trend-based insights, prioritize high-risk patients, and drive timely interventions to prevent decline
Document outcomes, refine protocols, and lead brief huddles to sustain adoption and measurable impact
Efficiently complete visits across designated SNFs (typ. 25+/day where appropriate), focusing on new admits, facility-flagged patients, and Circadia alerts
Surface field insights to improve workflows, summaries, and flagging logic; participate in root-cause reviews
Support NP onboarding, share best practices, and partner with clinical ops/product teams
Cover nearby sites (30–40-mile radius) as needed; mileage and lodging covered per policy
Qualification
Required
Certified and licensed Nurse Practitioner in New York; ANCC (or equivalent) in Adult/Family/Geriatric/Acute
Eligible for Medicare participation; company bills on your behalf
2+ years NP experience with complex adult populations; able to practice independently
Valid New York driver's license; comfortable traveling locally
Tech-savvy: Google Suite, multiple EMRs, Slack, AI transcription/tools
Thrives in a dynamic, evolving startup environment
