Responsibilities

Navigate the electronic medical record to evaluate patient care needs
Identify, resolve, and prevent potential and actual medication-related problems including untreated conditions, improper drug selection, sub-therapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication
Serve as a drug information resource for providers as well as nursing and other support staff
Utilize pharmacy consults and collaborative practice agreements to assist providers in meeting disease state goals for their patients
Apply continuous glucose monitors (CGM) and educate patients on their use at assigned clinic(s) to help patients monitor and report blood glucose levels
Review and respond to medication questions/concerns from patients at the clinic, via telephone, or electronically through My Sanford Chart
Complete medication reconciliation as requested by the clinic support staff
Conduct Office Visits or Medicare Annual Wellness Visits at assigned clinic(s)
Assist patients who cannot afford their medications by utilizing copay cards, patient assistance programs, formulary alternatives, etc. in both the clinic and pharmacy
Participate in Sanford Health Pharmaceutical Service relevant meetings to ensure practice and strategy alignment with various service lines
Collaborate and closely communicate with Sanford Providers/support staff to improve patient and provider satisfaction through efficiencies and improving processes
Review prescriptions to assure accuracy, to ascertain the needed ingredients, and to evaluate their appropriateness
Precept and mentor pharmacy students and pharmacy residents
Understand and adhere to billing regulations and compliance

Required

Graduate of an accredited School of Pharmacy
Must have a thorough knowledge of drugs and their actions
Knowledge of state and local regulations
