Quality Perf Improvement Coord / Quality Management
Hartford HealthCare is Connecticut’s most comprehensive healthcare network, and they are seeking a Performance Improvement Coordinator. This role involves performing systematic and objective performance improvement activities for both clinical and non-clinical areas, including data abstraction, analysis, reporting, and administrative support for the Quality/Regulatory/Patient Safety Team.

Responsibilities
Assists in performance improvement activities to monitor the quality of care to meet regulatory standards
Performs specific performance improvement activities necessary for the clinical and administrative staffs to monitor and evaluate the quality and appropriateness of patient care to meet regulatory requirements (e.g., DPH, TJC, CMS)
Provides data abstraction from hospital applications and inputs into internal and external databases to support publicly reported measures and other performance improvement activities. Coordinates medical record audits for the state, federal, and/or managed care review
Interprets regulatory specifications of Core Measures and accurately oversees CMS validation requirements
Generates analytical reports with statistical quality control tools for specialized and generic performance improvement activities
Coordinates ongoing screening, collection, and analysis of information
Identifies problems that have an impact on patient care and the clinical performance of members of the hospital staff
Provides administrative support for Quality/Regulatory/Patient Safety including support for meetings: including but not limited to scheduling, coordinating agendas, taking minutes, compiling day of final presentations, distributing pre-meeting packets, copying meeting materials, preparing meeting minutes. Meetings include Regulatory, Patient Safety, PICC, QAPIC, SERT, MSEC
Facilitates RAVE and internal communications for regulatory activity. Reviews and updates RAVE notification list quarterly
Creates reports including EPIC daily census, OR procedure lists, restraint report and supports ongoing regulatory activities. Provides support in scanning regulatory activity files into shared drive, research policies and IFU’s from online resources
Qualification
Required
Minimum: Associate Degree (desired but not required)
Minimum: One to three years of experience in the medical record or equivalent medical terminology knowledge
Strong interpersonal skills with a focus on customer service
Microsoft Office proficiency in Word, PowerPoint, Excel and Access
Able to acquire skills in data tools such as control chart, Pareto chart, trending data and analytics
Excellent written and verbal communication skills
Preferred
Preferred: Bachelor's Degree (desired but not required)
Preferred: Knowledge of Microsoft office products including word, excel, powerpoint ; quality improvement methods, tools and techniques
Preferred: CPHQ or other- optional
