Responsibilities

Support and coordinate the mental health care of patients
Monitor and track treatment response for changes in clinical symptoms and treatment side effects or complications
Support and encourage individuals and family members to increase knowledge about their mental and physical health conditions, engage in their care, and improve adherence to treatments
Provide education and available treatment options
Facilitate referrals for other needs and support patient medication management including monitoring adherence and ability to access obtained medications
Assist with managing patient appointments and phone calls for mental health care team, in coordination with support staff
Provide support to patients and families where psychosocial barriers interfere with their ability to access or maintain recommended treatments for behavioral health concerns

Required

Bachelors degree in Psychology or a related field and two years of healthcare-related experience
Bachelors degree in Nursing, with NYS RN licensure/registration (or eligibility) and two years of post-RN experience
Masters degree in Social Work from a CSWE-accredited program, with LMSW, LCSW, or eligibility, and three years of relevant post-masters experience (preferably in a hospital and/or healthcare setting)

Preferred

Experience in behavioral healthcare setting working with a multidisciplinary team and/or medical setting
Demonstrated prior interactions with patients in medical setting
