Responsibilities

Supporting and coordinating the mental health care of patients
Monitoring and tracking treatment response for changes in clinical symptoms and treatment side effects or complications
Supporting and encouraging individuals and family members to increase knowledge about their mental and physical health conditions, engage in their care, and improve adherence to treatments
Providing education and available treatment options
Facilitating referrals for other needs and supporting patient medication management including monitoring adherence and ability to access obtained medications
Assisting with managing patient appointments and phone calls for mental health care team, in coordination with support staff
Providing support to patients and families where psychosocial barriers interfere with their ability to access or maintain recommended treatments for behavioral health concerns

Qualification

Bachelors in PsychologyBachelors in NursingMasters in Social WorkNYS RN licensureLMSWLCSWBehavioral healthcare experienceMultidisciplinary team experiencePatient interaction experience

Required

Bachelors degree in Psychology or a related field and two years of healthcare-related experience
Bachelors degree in Nursing, with NYS RN licensure/registration (or eligibility) and two years of post-RN experience
Masters degree in Social Work from a CSWE-accredited program, with LMSW, LCSW, or eligibility, and three years of relevant post-masters experience (preferably in a hospital and/or healthcare setting)

Preferred

Experience in behavioral healthcare setting working with a multidisciplinary team and/or medical setting
Demonstrated prior interactions with patients in medical setting preferred
