Responsibilities
Provide inpatient consultation to patients with serious illness seeking assistance with advanced symptom management, clarifying goals of care, addressing end of life issues and care, and transition planning
Consults will be provided to all types of admissions including medical, surgical, and ICU as well as in the ED and will augment care provided by the medical and surgical teams
Work directly with the patient and their families and caregivers, and their hospital and specialty providers, to create a customized plan of care
Participate in shaping the education and training and improving care model through active participation in process and quality improvement
Promote awareness, knowledge, and skill in delivering palliative care throughout the organization
Qualification
Required
Ideal candidate will have completed a Palliative Medicine Fellowship Program
BE/BC in Palliative Care
All applicants must possess a full range of clinical skills and an interest in working with an innovative faculty practice
Experience working in an interdisciplinary team delivering Palliative Care Medicine
All applicants must have an unrestricted New York State Medical License and Federally Issued DEA license
