Responsibilities

Manage the admission process, including initial screenings, assessments, and coordination with interdisciplinary teams and intake

Develop and maintain relationships with referral sources, including physicians, hospitals, and other healthcare providers

Ensure compliance with regulatory requirements, including Medicare and Medicaid guidelines

Collaborate with clinical teams to ensure seamless transitions and high-quality patient care

Analyze admission data and trends to identify opportunities for improvement

Develop and implement strategies to increase admissions and grow the hospice program

Provide leadership and guidance to admission staff, including training, mentoring, and performance management

Foster a culture of compassion, empathy, and respect within the admission team

Participates in the on-call rotation

Provide education/inservice to internal and external customers about hospice philosophy and admission criteria

Do ride-alongs with Sales representatives

Shadow admission nurses on a regular basis

Visit facilities to do follow-up on how admission met and improve admission process as needed
