The Colorado Coalition for the Homeless is dedicated to preventing homelessness and creating lasting solutions for at-risk individuals and families in Colorado. The Pharmacist will provide comprehensive pharmaceutical care services, assist in medication therapy management, and ensure pharmacy operations are accountable and effective.

Responsibilities
Performs initial evaluative and final interpretation of electronic medication orders for appropriateness
Performs a comprehensive review of patient’s drug therapy profile for allergies, contraindications, drug-drug interactions, drug-food interactions, appropriateness of dose and drug therapy
Navigates through multiple electronic health systems in order to verify patient eligibility and process medication orders
Advocates and supports the development of appropriate pharmacy policies and procedures, together with participating in continuous quality improvement programs
Maintains a work environment that is marked by respect for others; which values inclusiveness and builds workforce diversity; which fosters cooperation, collegiality, communication, and teamwork
Provides clinical pharmacy services to patients including but not limited to anticoagulation services, immunization, and medication therapy management
Performs other duties as assigned by pharmacy/clinic management
Qualification
Required
Ability to multi-task and perform with a high degree of accuracy in a high-volume pharmacy work environment
Understands and confident in direct patient and provider consulting
Sound critical thinking and decision-making skills
Knowledge of homeless issues and demonstrated sensitivity to and a desire to work with underserved populations
Ability to work in a fast-paced environment with frequent interruptions
Must have a valid Colorado Driver's License and meet CCH automobile insurance eligibility
Doctor of Pharmacy degree and/or Bachelor of Science in Pharmacy
Current licensure in the State of Colorado as a registered pharmacist in good standing with the Colorado State Board of Pharmacy
Preferred
Prior clinical pharmacy experience
Bilingual (English/Spanish)
