Responsibilities

Deliver Compassionate, Comprehensive Care – Perform thorough assessments, develop individualized care plans, and provide expert nursing interventions that ensure comfort, dignity, and quality of life for patients and families

Lead and Coordinate Patient Care – Serve as Case Manager for assigned patients, coordinating interdisciplinary care and guiding families through the hospice journey with skill, empathy, and professionalism

Communicate with Purpose – Act as the vital link between patients, families, physicians, and the hospice team—ensuring seamless updates, education, and collaboration to achieve the best outcomes

Demonstrate Clinical Excellence – Administer medications and treatments, provide preventive and rehabilitative nursing procedures, and empower patients toward greater self-sufficiency and comfort

Grow, Lead, and Contribute – Participate in on-call and holiday coverage, mentor and delegate to support staff, and engage in continuous professional development and quality improvement initiatives
