Responsibilities

Prepare patients for physician visits

Connect with patients, ensure video and audio work, and add interpreters as needed

Obtain signed telehealth consent forms

Obtain photos of insurance cards

Take medical information over the phone

Document up to date medication list with dosing

Document Medical History

Document Primary Complaint

Answer incoming calls

Schedule follow up appointments

Schedule labs

Engage in confirmation calls for upcoming appointments (24 hrs in advance)

Perform chart checks

Engage in chart prep prior to patient visits (24 hrs in advance)

Discover and recommend better ways to communicate, connect, and build relationships with Heartbeat patients

Proactively identify opportunities for improvement and risk mitigation

Execute policies that promote the best patient experience
