Responsibilities
Provide Compassionate Personal Care – Support patients with daily needs such as bathing, grooming, dressing, mobility, nutrition, and comfort measures, ensuring dignity and quality of life at every stage
Promote Safety and Well-Being – Use equipment properly, assist with ambulation and exercises, observe and protect skin integrity, and report significant changes in condition to the care team
Support Families with Homemaking Services – Maintain a clean, safe, and orderly home environment by assisting with laundry, meal preparation, light housekeeping, and shopping as needed
Collaborate and Communicate – Work closely with RN Case Managers, therapists, and caregivers to provide seamless care, accurate documentation, and emotional support for patients and families
Contribute to Excellence in Care – Participate in quality improvement activities, follow organizational standards, and consistently demonstrate professionalism, compassion, and respect in every interaction
Qualification
Required
High School Diploma or equivalent
Completion of recognized Nursing Assistant training
1-year full-time experience in a hospital or nursing facility: hospice experience preferred
Current LA Driver's License
Automobile Liability Coverage
CPR Card
