Qualification
Required
Master's Degree (MA/MS) or equivalent, or zero to five years of related experience or training, or equivalent education and experience
Maintains current licensure by the state of Arizona and be certified by the Board of Certification
BLS certification is required
A valid state driver's license and proof of insurance
Contractor shall provide and maintain liability insurance from a company approved by Source Management for general and professional liability in an amount no less than $1,000,000/$3,000,000
Contractor shall provide a certificate of insurance to such effect, which shall further provide that Source Management shall receive at least 15 days prior notice before any cancellation, non-renewal or modification in terms of such insurance
