The Licensed Professional Counselor (LPC) will provide individual and group psychotherapy, develop treatment plans, and collaborate with facility staff to enhance patient care.

Responsibilities

Serve as a seasoned social work consultant in long-term care facilities
Provide individual and group psychotherapy sessions
Complete mood and symptom-based screenings
Develop individualized treatment plans for adults and/or seniors
Provide in-service education
Collaborate with facility staff and consult with interdisciplinary teams
Maintain accurate, compliant documentation

Qualification

Licensed Professional CounselorDegree in Social WorkSevere mental illnessActive state licenseCultural humilityCommunication skills

Required

Master's Degree in Social Work
Licensed Professional Counselor with the ability to practice and bill independently
Active license to practice in State(s) where MediTelecare operates
Significant experience in treating adult and/or senior patient population, particularly with patients exhibiting severe persistent mental illness
Strong communication skills and cultural humility
