Responsibilities

Provides drug information services by responding to questions regarding medication compatibilities, adverse drug events, regimens, dosing, interactions, storage and disposal, compounding, therapeutic uses of medications and pharmaceutical supplies in a timely manner; using a systematic approach to effectively searching, retrieving, and evaluating the literature and appropriately communicating and applying the information to the patient care situation; and assisting with the development, maintenance, and implementation of hospital and department policy and procedures related to medication use, storage, restrictions, and administration
Sustains and implements quality and safety initiatives and practices in the department and throughout the hospital. Reports adverse drug events and medication variances using the hospital variance reporting program (QSRS)
Sustains clinical pharmacy services throughout the organization by providing pharmaceutical care for pediatric patients of all age ranges through participation in multi-disciplinary team rounds and/or care, emergency response, education, guidance, interpretation and analysis of medication related therapeutic issues; documenting pharmaceutical interventions according to departmental standards; performing therapeutic drug monitoring and disease outcome analysis; and timely verification of medication orders in an electronic health record system
Participate in general pharmacy operations throughout the organization through appropriate medication preparation, dispensing, and inventory control. Provides oversight to pharmacy technicians, interns, residents, and students as needed

Qualification

Pediatric pharmacy experienceAmbulatory care experienceDoctor of PharmacyBLS certificationPharmacist license

Required

Doctor of Pharmacy or Bachelor of Science in Pharmacy degree
Completion of one year pharmacy residency
BLS certification (American Heart Association, American Red Cross, or American Safety and Health Institute accepted)
State of Colorado Department of Regulatory Agencies (DORA) Pharmacist license

Preferred

At least one year of pediatric pharmacy and/or ambulatory care experience
Completed a residency in pediatrics or ambulatory care
One year of pharmacy work experience accepted in lieu of residency
