Responsibilities
Designs, recommends, monitors, and evaluates patient specific pharmacotherapeutic regimens when consulted
Provides accurate information on medication use, possible side effects and their management, drug interactions or other needed facts to patients and their caregivers
Provides accurate, timely drug information to other healthcare providers
Assesses, manages, prevents, and reports medication errors and adverse reactions to medications
Prepares and dispenses medication using appropriate techniques and following departmental policies and procedures
Ensures that the appropriate medication and dosage is used, with appropriate monitoring for clinical effects and the absence of adverse effects
Provides drug information in an accurate and timely manner
Works with other members of the healthcare team to continually improve the medication use process and the drug distribution system so patients may benefit fully from their medications
Manages workflow well, utilizing departmental staff to achieve goal turnaround times
Maintains a commitment to the continued development of the profession of pharmacy and its practitioners
Qualification
Required
Actively enrolled in an accredited school of pharmacy
Registered with Kentucky Board of Pharmacy as a Pharmacist Intern
