Responsibilities

Performs rounds with the Critical Care Unit and other Inpatient Care Units, as required. Conducts ongoing review and education on dialysis nursing care to staff including all shifts. Ensures elements of the Acute Dialysis program are in place and functioning appropriately. Acts as clinical resource to staff by providing in-services, validating dialysis-related competencies and advising management of training needs
Coordinates all disciplines involved in the care of patients to ensure appropriate, cost-effective and efficient progression through the system and continuity of care. Leads multidisciplinary team efforts to identify clinical problems and needs. Designs processes to implement and manage patient transitions through the system. Evaluates care to ensure desired patient outcomes. Works under minimal supervision
Serves as the point-of-contact for the program to both internal/external customers and healthcare providers. Maintains and ensures continued success of the program by complying with quality and regulatory requirements, as outlined by the appropriate agencies. Functions as an expert clinician, consultant educator and leader in the care of the dialysis patient population. Implements day-to-day care management interventions for identified patients
Mentors, develops, inspires, educates and leads staff members in patient care setting. Maintains responsibility for enhancing efficiency and cost effectiveness by identifying opportunities to decrease costs, as applicable to the job. Demonstrates subject matter expertise regularly regarding medication, treatment and patient care standards

Qualification

Registered Nurse (RN) licensureBachelor's Degree in NursingCritical Care Registered Nurse CertificationExperience in acute care nursingExperience in cardiovascular nursingExperience in orthopedic case managementMaster's Degree in NursingCertification in case managementCertification in orthopedic nursing

Required

Registered Nurse (RN) licensure in the state of practice

Preferred

Bachelor's Degree in Nursing (BSN)
Master's Degree in Nursing, Clinical Nurse Specialist or Nurse Practitioner
Experience in acute care nursing
Experience in cardiovascular nursing
Experience in orthopedic case management
Critical Care Registered Nurse Certification (CCRN)
Certification in case management and/or orthopedic nursing practice
