This hybrid leadership role combines administrative oversight with nursing supervision, ensuring high-quality, person-centered care for clients while maintaining regulatory compliance and operational excellence.

Responsibilities
Provide leadership for agency operations including staffing, client services, compliance, and quality assurance
Develop, implement, and maintain agency policies and procedures
Oversee hiring, training, evaluating, and supervising Personal Care Workers (PCWs) and clinical staff
Maintain secure and accurate records of staff, clients, and operations
Conduct initial client assessments and develop individualized care plans
Perform supervisory visits every 60 days (or sooner if needed) to evaluate client care and PCW performance
Train, support, and oversee PCWs, providing clinical guidance and delegation as appropriate
Communicate and coordinate with physicians, families, and other healthcare providers
Ensure all documentation meets DHS and Medicaid standards
Monitor and audit service delivery to ensure compliance with DHS regulations and Medicaid requirements
Lead quality assurance and improvement initiatives
Advocate for clients, address concerns, and ensure safe and effective care delivery
Qualification
Required
Current Registered Nurse (RN) license in Wisconsin (Wis. Stat. §441.06)
BSN required
Minimum 1 year of supervisory experience in personal care, home health, or a related healthcare program
Training and experience in healthcare administration and personal care services
Knowledge of Wisconsin Administrative Codes DHS 105.17 and DHS 107.112
Strong leadership, communication, organizational, and problem-solving skills
Must be at least 21 years of age
