They are seeking a Full-Time Functional Medicine Nurse Practitioner / Physician Assistant who will provide primary care services, develop treatment plans, and educate patients on health management, while working within a multidisciplinary team.

Responsibilities
Conducting patient assessments and providing primary care services, including diagnosis, treatment, and follow-up care
Performing Hormone replacement therapy
Developing treatment plans in collaboration with physicians and other healthcare providers
Educating patients and their families on disease prevention and management, healthy lifestyles, and medication use
Documenting patient care in electronic medical records (EMRs) and maintaining accurate patient records
Collaborating with the Medical Director and other healthcare providers to ensure continuity of care
Participating in quality improvement initiatives and staying up-to-date on best practices and clinical guidelines
Evaluating, diagnosing, and recommending individualized treatments or products
Ordering and interpreting labs, including blood tests and other diagnostics
Proposing tailored treatments based on patient history, conditions, and concerns
Maintaining precise records and schedules for efficient workflow
Staying current with advancements in the field and engaging in ongoing research
Collaborating with a multidisciplinary healthcare team to ensure comprehensive patient care
Performing patient assessments and developing treatment plans in conjunction with physicians and colleagues
Ordering and interpreting diagnostic tests and prescribing medications as necessary
Educating patients and families on disease prevention and management strategies
Keeping thorough and accurate patient records using electronic medical records (EMRs)
Participating in quality improvement initiatives and staying abreast of best practices and clinical guidelines
Qualification
Required
A Master of Science in Nursing (MSN) degree and certification as an Advanced Practice Registered Nurse (APRN)
A valid state license to practice as a Nurse Practitioner
3+ years of experience as a Nurse Practitioner in primary care or a related field
3+ years of Functional Medicine experience
Excellent clinical and diagnostic skills
Previous experience with hormone treatments
Strong communication and interpersonal skills
Ability to work collaboratively as part of a team
Familiarity with electronic medical records (EMRs) and other clinical software
