They are seeking a Family Model Provider to offer a safe and comfortable home for individuals with developmental and/or physical disabilities, ensuring their basic physical and emotional needs are met.

Establish a comfortable, supportive, and genuine relationship with the individual you house and support
Qualification
Required
High School Diploma/GED
Valid Driver's License
Previous experience working as a Direct Support Professional is required
Will need to successfully complete trainings through Community Options
Must be open to supervisory visits in your home
