Designated Coordinator
BrightPath LLC is seeking a compassionate and organized Designated Coordinator to support individuals with disabilities and older adults in community-based residential programs. This role involves overseeing service delivery, ensuring compliance with support plans, and providing guidance to staff while acting as a liaison between various stakeholders.

Responsibilities
Provide oversight and coordination of services based on individual support plans
Train, support, and guide direct support staff
Ensure compliance with all 245D regulations, documentation standards, and internal policies
Monitor health, safety, and progress of individuals served
Lead team meetings and support goal planning for individuals
Collaborate with families, case managers, and healthcare providers
Maintain accurate documentation, incident reports, and progress summaries
Participate in on-call rotation and provide occasional direct care as needed
Qualification
Required
Bachelor's degree in a human services field + 1 year of full-time direct care experience
Associate's degree in a human services field + 2 years of full-time direct care experience
Diploma in a human services field + 3 years of full-time direct care experience
4 years of full-time direct care experience under qualified supervision
At least 50 hours of documented training/education related to human services and disabilities
Supervisory or team leadership experience in social services
Strong working knowledge of Minnesota 245D licensing requirements
Valid driver's license and reliable transportation
