They are seeking a Palliative Care Nurse Practitioner to provide specialty outpatient care, oversee interdisciplinary case conferences, and educate team members on palliative care best practices.

Responsibilities

See members for specialty palliative care outpatient appointments via video visits as part of an interdisciplinary team
Responsible for the oversight and direction of interdisciplinary case conferences for members with advanced serious illness
Provide education about palliative care to the team of Community Health Workers and Oncology Nurses

Qualification

Palliative care experienceNurse Practitioner licenseACHPN certificationOutpatient oncology experienceInterdisciplinary collaborationComfort with ambiguityTechnology proficiencyEffective communicationOrganizational skills

Required

At least 3 years of nurse practitioner (NP) experience
At least 2 years in palliative care, preferably caring for patients in the outpatient oncology setting
Advanced certified hospice and palliative nurse practitioner (ACHPN)
Unrestricted nurse practitioner license
Willingness to obtain additional state licenses, as needed
Skilled in juggling multiple tasks and working under pressure without sacrificing organization in communications and documentation
Effective listener and communicator
Comfort with ambiguity in fast-paced environments
Desire to learn how to use new technologies
Experience with video chatting, Google Suite, Slack, electronic health records or comfort using and learning new technology
Ability to identify and prioritize a member's needs

Preferred

Experience working at a startup or tech-forward company
