The Patient Navigator plays a critical role in supporting Health Center patients throughout their care journey by coordinating diagnostic testing and ensuring seamless follow-up with outside entities.

Responsibilities

Outreach patients to schedule necessary appointments

Assist in overcoming barriers to care such as transportation or financial challenges

Retrieve diagnostic and referral results to ensure comprehensive documentation and continuity of care

Monitor, update, and follow up on patient referrals using external referral management systems to ensure patients are scheduled with outside specialists, services, or community partners

Address patient navigation needs flagged within the internal electronic work queues, providing a high level of support and guidance to patients throughout the care continuum

Identify and Reduce Barriers to Care

Provide support directly or connect them to community-based resources
