Qualification

Doctoral degree in PsychologyLicensed Psychologist in IndianaHealth Service Provider endorsementChildrenAdolescentsReliable transportation

Required

Doctoral degree and licensed in the state of Indiana as a Psychologist
Endorsed as a Health Service Provider in Psychology (HSPP), by the Indiana State Board of Examiners in Psychology
Valid driver's license, a driving record meeting Meridian's driving policy, reliable transportation, and proof of auto insurance required
