Responsibilities
Oversees the daily activities ensuring department standards are met, and provides real-time mentoring, advisement, and education to CM staff following CM processes
Works with the CM Manager on the day-to-day activities of the department, including but not limited to assisting in creating staff schedules and ensuring adequate coverage
Quality and productivity monitoring of staff work and documentation in EMR and EMR tools. Guide staff toward desired outcomes, setting high-performance standards and delivering quality services
Qualification
Required
Education: Master of Social Work
Licensure: LMSW, LCSW or LCSW-C
Experience: 3-5 years of acute care hospital Case Management experience required
Preferred
Certification: Case Management certification preferred
