Giving Home Health Care is a leading provider of personalized, in-home care for individuals affected by health conditions related to their work in nuclear facilities. They are seeking a compassionate Nurse (RN or LPN) to administer patient care in accordance with physician-established care plans, advocate for patients, and collaborate with other healthcare professionals to ensure effective treatment and support.

Responsibilities

Execute physician prescribed plans of care

Complete compliant documentation of the care provided in the system of record

Manage and administer prescribed medication, treatment, and therapies

Conduct patient assessments, coordination of care

Perform various activities associated with daily living

Monitor vitals and GI intake and output

Assess skin integrity and administer wound care

Monitor for changes in the patient’s condition such as weight loss/gain, self-care abilities, and indicators of disease progression

Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients

Provide education and training to the patient, their family, and/or the Home Health Aide

Work with patients to be done one-on-one in the patients home to provide for a comfortable environment

Collaborate with and supervise Certified Nursing Assistants and Personal Care Attendants

Perform other personal care services as necessary to meet the patient’s needs

Required

Registered Nurse (RN) or Licensed Practical Nurse (LPN) certification

Ability to execute physician prescribed plans of care

Experience in completing compliant documentation of care provided

Skills in managing and administering prescribed medication, treatment, and therapies

Capability to conduct patient assessments and coordinate care

Experience in performing various activities associated with daily living

Ability to monitor vitals and GI intake and output

Skills in assessing skin integrity and administering wound care

Ability to monitor for changes in the patient's condition such as weight loss/gain, self-care abilities, and indicators of disease progression

Strong communication skills with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients

Experience in providing education and training to the patient, their family, and/or the Home Health Aide

Experience in collaborating with and supervising Certified Nursing Assistants and Personal Care Attendants
