They are seeking an Internal/Family Medicine Physician to provide primary care services, conduct patient assessments, and support transitional care in a collaborative environment.

Responsibilities
Provide primary care services to a diverse adult population, with a focus on LGBTQ+ and gender-affirming care
Conduct thorough patient assessments and develop individualized, evidence-based care plans
Deliver hormone therapy management and support transitional care in coordination with specialists as needed
Maintain accurate and timely documentation
Participate in a highly collaborative care team focused on long-term patient relationships
Qualification
Required
MD or DO with board certification/eligibility in Internal Medicine or Family Medicine
Active or eligible for New Jersey medical license
Experience or interest in LGBTQ+ healthcare, including gender-affirming care and transitional medicine
Excellent communication and interpersonal skills
Commitment to compassionate, inclusive, and culturally sensitive care
