Responsibilities
Provide care to a complex patient panel
Manage chronic diseases
Collaborate with dietician, social worker, and pharmacist
Utilize EMR – EPIC
Oversee onsite lab and X-ray services
Qualification
Required
MD or DO degree
Board certification or eligibility in Internal Medicine
Experience in managing complex patient needs and chronic disease management
Ability to work in a primary care setting
Familiarity with EMR systems, preferably EPIC
Strong communication and interpersonal skills
Commitment to providing high-quality patient care
Preferred
Experienced candidates preferred
