The Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) will deliver holistic care in long-term care facilities, collaborating with specialty clinicians and coordinating care plans with families and other healthcare providers.

Responsibilities

Provide evaluation

Provide condition-specific support therapy, or when indicated, insight-based therapy

Coordinate the patient’s care plan with the facility, PCP and GuideStar colleagues, and engage with the family as indicated

Qualification

Licensed Clinical Social WorkerLicensed Professional Counselor’s degree in Mental Health Counseling’s degree in Social WorkElectronic Medical Record (EMR) systemInterdisciplinary treatment teamGeriatrics passionInpatient settings experienceBrief bedside assessmentTime management

Required

Ability to work as part of an interdisciplinary treatment team

Good time management skills

Able to function autonomously with regional Area Manager support

Master's degree in Mental Health Counseling or in Social Work

Currently licensed as an LMHC or LCSW in the state in which you are applying

Preferred

Passion for geriatrics

Experience in inpatient settings such as extended care facilities

Experience with brief bedside assessment screens (cognitive, mood, etc.)

Motivated by productivity-based positions

Ability to navigate and document on an Electronic Medical Record (EMR) system
