Responsibilities
Primary Care practice
Complex patient panel
Lean Management
Large patient population w/ chronic disease management
EMR – EPIC
Dietician, Social Worker and Pharmacist rotate through clinic and available virtually
Onsite Lab and X-ray
Qualification
Required
MD or DO degree
Board certification in Internal Medicine
Experience in complex clinical management and chronic disease management
Ability to work in a primary care practice
Willingness to consider new graduates
Ability to work with a complex patient panel
Familiarity with EMR systems, specifically EPIC
Preferred
Experienced candidates preferred
