The Hospice Liaison or Account Executive will coordinate patient referrals, establish relationships with local hospitals, and educate the community on hospice services to ensure continuity of care.

Responsibilities
Coordinating & generating new hospice business in both existing and new accounts
Identifying new markets and maintaining existing service
Coordinating care for referred patients
Serving as a liaison between the agency, hospitals, medical community and other referral sources
Educating the community and the medical profession/referral sources regarding hospice services
Daily interaction with patients, medical professionals, other referral sources, and the community to assure continuity of care
Coordinating appropriate communication and documentation
Working closely with agency personnel to ensure that patient care is handled efficiently and effectively
Engaging away from the employee’s place of business in performing these duties
Qualification
Required
A heart to serve patients and families and a passion for providing the best possible care
Education: Minimum of an Associate Degree required in field of study or proven work experience in health related field
Experience: 2 year of sales experience in a clinical care setting, hospice preferred
Required: Reliable transportation
Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively
